Health services

Integrated health services

If you or your loved one live with challenging health needs, you already know how difficult it can be to find the right help and support. Often you need more than one type of professional and it’s usually up to you to pass on the right information from one person to another. Disjointed care is not only tiring and time-consuming, but it’s also less than you deserve.

The Talbot Centre offers seamless, integrated care. We bring together a carefully chosen team of healthcare professionals under one roof so that you can experience the benefits of connected, coordinated care across many areas of your health.

That transforms your experience of healthcare, making it far simpler and more convenient.

Disconnected Care

Disconnected care

(a common experience)

You are in the centre of this hub and you’ve engaged several professionals to help you. But they’re not connected to each other. They may provide great advice but the ‘wheel’ doesn’t turn well and that limits your progress.

Connected Care

You-centred care at
The Talbot Centre

You’re in the centre of this wheel too but there’s a crucial difference. The members of your healthcare team are not only connected to you – they’re also connected to one another, share the same underlying values and are committed to supporting you to work towards your goals. This wheel turns smoothly and you make progress.

Though you may only see one or two professionals, the care you receive at The Talbot Centre will often be informed by insights from others in our team. That’s because the members of our extensive multidisciplinary team all work closely with one another so that you receive consistent care working towards the same goals.

The Talbot Centre - seamless, integrated care.

On the same page and under one roof

Having your whole treatment team under one roof means you can access a range of services in one familiar place. At The Talbot Centre, you have access to a growing team of GPs, medical specialists and allied health professionals, including clinically trained psychologists, dietitians, speech pathologists and occupational therapists who work collaboratively with one another.

But location isn’t all that matters. You need a team who are all on the same page when it comes to your care. The Talbot Centre staff are more than skilled professionals – they were all carefully chosen because they embody our neurodiversity-affirming, trauma-informed and body-positive perspective.

Whilst many people choose to access the benefits of having their whole healthcare team within the one service, You will always be at the centre of your care. The Talbot Centre health professionals are happy to work collaboratively with external healthcare providers that you have selected to join your care team. We use our experience as an interdisciplinary team to facilitate the highest level of coordination and collaboration of care no matter where your healthcare team are located.

With your permission, the professionals involved in your care will hold regular case conferences where they meet and discuss your needs to ensure they’re giving you the most appropriate treatment.

A sustainable future for healthcare

The Talbot Centre is committed to ensuring that high quality, You-centred healthcare continues to be available long into the future.

As part of this commitment, our senior healthcare practitioners make an active contribution to nurturing the next generation of healthcare professionals.

Clinical placement forms an important part of their training, and we are pleased to enable students and interns to provide services under the guidance of our senior staff. This benefits you in both the short and long term.

Students and interns are not only learning from our professional team. They’re also learning from you – you have a deep, lived experience of the health conditions you manage on a daily basis. Many people appreciate having their expertise recognised like this and put to good use.

A sustainable future

Having an early career health professional involved in your treatment team can also facilitate access to high quality intervention at a significantly reduced overall cost. You can benefit from the wisdom of our senior clinicians, who will take an active interest in understanding your needs and supporting the development of a plan to address them, in the most time efficient and cost effective way.

Supporting the development of early career clinicians, students and interns increases the availability of informed and experienced professionals in the workforce, reducing the wait times for healthcare services, so that you and many other people can access the care you need at the time you need it.

Those trained at The Talbot Centre will hopefully embody our neurodiversity-affirming, trauma-informed and body-positive values and have a much broader knowledge base underpinning their provision of healthcare, due to their exposure to our interdisciplinary team throughout their training.

Many of our early career clinicians are permanent members of staff, however, our students are with us for a shorter time than our permanent staff members. We know this in advance and can advise you of the students planned length of engagement with the service, so you can make an informed decision about whether you would like to involve them in your care. When they move on, we carefully manage your transition to a new member of The Talbot Centre team.

As always, You are at the centre of your care at The Talbot Centre. Our triage team can assist you in determining whether involving a student or early career professional in your treatment team is well aligned with your needs.
